Dear Prospective Donor:
We greatly appreciate your interest in the anatomical donor program at the University of California, San Diego. It is only through the generous gifts of our anatomical donors that health professionals are able to advance their knowledge of medicine to the highest degree. You may be assured that we hold the privilege of studying the human body in sacred trust. All cremations are done here at UCSD in a dedicated crematorium and are carried out by Body Donation Program staff members. Remains are scattered at sea.
If you wish to register with our program, please complete the UCSD Donation Agreement, Vital Statistics form, State of California - Health and Human Services Agency/Department of Health Services Worksheet for Education and Race/Ethnicity, and UCSD Authorization for Cremation and Disposition.

** You may wish to keep copies for your personal files. It is not necessary to have the forms notarized. Once your enrollment has been verified, you will be sent an anatomical donor card to keep in your wallet or billfold. You can verify we have received your form by calling (858) 534 - 4536.
