Donor Application

We greatly appreciate your interest in the body donation program at the UC San Diego. It is only through the generous gifts of our anatomical donors that health professionals are able to advance their knowledge of medicine to the highest degree. You may be assured that we hold the privilege of studying the human body in sacred trust. All cremations are done here at UC San Diego in a dedicated crematorium and are carried out by Body Donation Program staff members. Remains are scattered at sea.

Please note:

All forms must be completed and returned by mail, fax, or email before enrollment in the program is complete. Forms are not able to be saved and must be printed.
You may wish to keep copies for your personal files.
It is not necessary to have the forms notarized.
Once your enrollment has been verified, you will be sent an anatomical donor card to keep in your wallet or billfold.
