Apply for Federal Student Loan Debt Relief

First-Time Borrower Forms & Master Promissory Notes (MPN)

If this is the first time you have received any of the school-based loans listed on your award letter you will you will notified via the loan servicer ECSI to view and E-sign an electronic version of the applicable Master Promissory Note (MPN).

As a first time borrower, if you are unable to complete the MPN electronically through ECSI, a PDF link is provided below. If you choose this option you will need to print, complete and return the MPN as a hard copy with a wet signature to the address listed below.

Submit your forms as a hard copy with wet signature to:

Students who would like to request additional Direct Loans may complete the form below and submit it through their Health Sciences Forms Portal Account.
